That's tough. Be ready for her feelings to be hurt, but you could always say that you and your FI decided on a small wedding party. You could always think of another job she could do though, so that she feels like she's part of the wedding.

Don't ignore the problem! I was the "left over" friend of our group, and it hurt not to be included, but even more that she didn't even approach me about it. Your friend shouldn't have assumed that you'll ask her to be a bridesmaid, but the best way to get past the problem is just to confront it.
